Trodelvy Dosage and Administration

 Note: It is not advisable to substitute Trodelvy injection for or use with other medicinal products that are known for containing irinotecan or its active metabolite SN-38.



Proposed Dosage of Trodelvy:

The proposed dosage of Trodelvy is 10 mg/kg. The dosage needs to be administered as an intravenous infusion once a week on the 1st & 8th Days of 21-day treatment cycles. Continue treatment with trodelvy 180 mg until the disease progresses or has unacceptable adverse reactions.

Avoid administering Trodelvy at doses of more than 10 mg/kg. The medicine Trodelvy is for intravenous use only. This medicinal product is not for use as an intravenous push or bolus. 

Initial Infusion: Administer trodelvy infusion over three hours. Assess respective patients throughout the infusion and a minimum of 30 minutes following the initial dose for signs-symptoms of infusion-related reactions. 

Subsequent Infusions: Administer trodelvy infusion over 60-120 minutes in case previous infusions were tolerated. Assess respective patients throughout the infusion and for at least 30 minutes after infusion.

Premedication:

Before each dose of Trodelvy injection, premedication to prevent infusion reactions and chemo-induced vomiting and nausea is recommended. 

  • Patients should be premedicated with antipyretics, H1 & H2 blockers before each infusion, and corticosteroids may be considered for all those who had earlier infusion reactions.
  • The respective patients should be premedicated with a 2 or 3-medicine combination regimen.
