Trodelvy: All about the drug



Indication and Usage

The medicine Trodelvy is a Trop-2-directed antibody and topoisomerase inhibitor conjugate drug, which is used in the treatment of adults suffering from locally advanced or metastatic triple-negative breast (mTNBC) cancer, HR+/HER2- metastatic breast cancer, and locally advanced or metastatic urothelial cancer (mUC).

  • It is used when it is not possible to remove cancer with surgery.
  • Trodelvy comes in the form of single-dose vials in 180 mg for intravenous infusion.

Dosage

  • Its dosage is based on body weight, and the recommended dosage is 10 mg/kg administered intravenously once weekly on the first day and eighth day of a repeating 21-day cycle.

Administration

  • First infusion: The drug is infused in the body for 3 hours, and during the infusion, the patient is observed for any adverse reaction caused by the infusion. After 30 minutes of infusion, observation should be done again to check for any post-infusion reaction.
  • Subsequent Infusion: If the drug is tolerated in the first infusion, then a subsequent infusion of 1-2 hours has to be done, and after 30 minutes and during infusion, the patient should be observed for any reaction.

Premedication: 

Before infusion, antipyretic drugs are given in order to induce nausea and vomiting, as they are common side effects.

Contradictions and Precautions

  • TRODELVY (sacituzumab govitecanshould not be substituted with other drugs containing irinotecan or its active metabolite SN-38.
  • Adverse drug reactions can encompass hypersensitivity reactions, including severe anaphylactic reactions. In life-threatening cases, TRODELVY is discontinued.
  • TRODELVY can cause embryo-fetal toxicity as it crosses the placental barrier, and it is also advised not to breastfeed.
  • People who are homozygous for the UGT1A allele are at much higher risk for neutropenia, febrile neutropenia, and anemia.

Adverse drug reactions

  • Fatigue and loss of appetite are common side effects.
  • Decreased lymphocyte and leukocyte count.
  • Decreased hemoglobin and anemia.
  • Increased glucose and alkaline phosphate salts.

Packaging and Storing

  • Trodelvy for injection is a sterile, off-white to yellowish lyophilized powder in a single-dose vial.
  • Each vial of Trodelvy is individually boxed in a carton, and each vial has 180 mg of the drug.
  • It is stored in dark and cool places.
